J.C. Bradfield (jcb@dcs.ed.ac.uk) has written a fully functional driver which converts PPM files ("ppmtomd"). He started hacking ghostscript, but abandoned it.
Dye sublimation option available.
The "md5k" driver appears to support 1200x600, 600, and 300dpi operation on a variety of papers. Unfortunately it is documented in Japanese.
